PTV - additional field template (photo, name, value)

ptv.tpl - the file responsible for forming an additional field.

Tag	Description
{title}	The name of one element from add. fields.
{value}	The value of one element from add. fields.
{photo}	Photo of one item with add. fields.
{counter}	A counter in order for each list.
[photo]...[/photo]	Displays content between tags if an image has been set.
