CTV - additional field template (color, name, value)

ctv.tpl - file responsible for forming an additional field.

Tag	Description
{title}	The name of one element from add. fields.
{value}	The value of one element from add. fields.
{color}	Color of one element with add. fields.
{counter}	A counter in order for each list.
